I understand the referring clinic will be billed for this procedure unless new bloodwork or advanced procedures are required to be performed.

I am a registered veterinarian and have personally performed a clinical examination of this patient within the past 14 days. I confirm that a pre-general anaesthetic blood panel has been completed within this timeframe and that the relevant results have been submitted with this referral. I understand that if current bloodwork (within 14 days) is not available, pre-anaesthetic blood testing will be performed by VECA on the day of the CT appointment, and all associated costs will be payable by the owner. I acknowledge that this referral is being submitted on behalf of the owner, and that the above requirements and potential additional costs have been discussed with them.
